Instructions for RESERVING TICKETS for Hot Air Balloon Rides:
- Please read Ballooning Procedures and Facts
- Complete and send the Balloon Ride Ticket Request Form
- Complete and send the Balloon Passenger Assumption of Risk and Release of Liability Form
- Include payment via check (payable to: Chautauqua Leadership Network)
- Mail check and both completed forms mentioned above to:
